(Please note: normal office hours are Monday through Friday 8:00 am to 5:00 pm, if you submit your request after 1:00 pm, you may not receive a call until the following business day). This appointment request page is not for same day appointments. If you wish to schedule a same day appointment please contact our scheduling department at 281-586-3888 option 1.

You may also visit our walk-in/extended care clinic if waiting for an appointment does not fit your schedule.
